The Illinois Department of Human Services and the Illinois State Diabetes Commission have launched an online survey to gain additional information about diabetes in Illinois.

The survey is designed to identify needs or barriers regarding diabetes prevention and control and is available at www.dhs.state.il.us - Results of the polls will be used to help formulate the state's diabetes prevention and control plans.

In Illinois, more than 840-thousand adults have been told by their doctors that they have diabetes. The IDHS overall mission is to assist everyone in achieving maximum self-sufficiency, independence and health.
